I have never seen or heard about the 'mispressed' laserdisc I have. It's a copy of 'The Warriors' (LV1122) but it's not the movie 'The Warriors' but 'Charlotte's Web' (I think LV8099 - both from Paramount Home Video)! Also strange is that the Laserdisc doesn't have any mint marks on the inner cicrcle. The label on the LD also says 'The Warriors'.
So, is this a unique copy and rare, or is this common?

It's not unheard of for the wrong label to be applied to a disc, after which it would almost certainly go into the sleeve matching the label.

The mint marks are probably on the outer edge of the disc and backwards. Both of these titles were in the first wave of Paramount discs released in the summer of 1981. They were pressed in Carson at the Discovision plant and this sort of thing happened with a lot of Discovision titles.

I've had a few 2 disc like this, one with a different part of two movies on each side.
