I have a good copy with the insert but no Obi. I am willing to pay $15 shipped.

i don't have that disc or obi but somone could.
i had a disc in the past that came with 2 obi strips, it was new and sealed so it could happen that there is an extra somewhere.

I had a copy of Eraserhead with 3 once, that was pretty neat.
