I have been trying to remember the issue with My Fellow Americans *DEFECTIVE* (1996) [14535] that prompted the recall and reissue under My Fellow Americans (1996) [15604]. Can anyone remember?

I know that "The Fly" and "The Glimmer Man" were recalled due to audio sync issues with the AC3 track.
"I Know What You Did Last Summer" was a recalled for a trailer issue
"Hunt for Red October" was recalled for silent surrounds on the AC3 track on side 3
"Stargate" had the Digital tracks out of phase which caused Pro-Logic decoding errors
"The Exorcist" had the audio commentary audio mixed too low.
