Looking at KILA 9401~4 and KFOLA 157, is the only difference between the two is the latter has a different reference number? If so, does that make it a re-release?

There is a thread somewhere on this forum that discusses KFOLA releases, maybe you can find some good information there.
In general: It is identical, but I'm not sure we ever found out what KFOLA releases were meant to be.

KILA = King Records / Star Child (most common)
KFOLA = Kadokawa / Star Child (shows up once in a while)

Some region/copyright issues? Was Okinawa separate from the rest of Japan or something like that?

What is that logo on the back wrap next to the Star Child logo? It’s too low res for me to read.

It's Kadokawa Shoten (角川書店)

They publish NewType, maybe they had a special batch for their subscribers?
