I have a couple thousand discs and 10 players now, lol. I will have to go through your list. The first player I had was a magnavox. Got it for 200 bucks in 1990. Most valuable disc I have owned was South Park the Movie.

5)   Best Sounding Disc - Dolby Digital AC-3 Wings of Honneamise Memorial Box (JP rerelease)

6)   Best Sounding Disc - DTS don't have any!
7)   Worst Sounding Disc (Any sound format, not laser-rot related) Area 88 Act 3 (US) — sounds like telephone audio

8)   Favorite All-Around Demo Disc (Audio and video presentation)
9)   All-Around Worst Disc You Own (Not laser-rot related)
10) Best Package Art
11) Best Gatefold Jacket
12) Worst Package Art Or Gatefold
13) Best Box Set The Japanese KOR TV box — each disc is in an individual sleeve, & 1-6 & 7-12 are then in half-boxes which slide into the outer box, & each half-box has a cloth tab on it, to pull it from the outer box with. I really wish more sets had followed that example, because sliding the single, huge inner box out of, say, the Maison Ikkoku TV box is very difficult.

14) Worst Box Set The AnimEigo Urusei Yatsura TV set, hands down the worst packaging of any LD I have. Its other faults could be borne if the discs didn't stick so firmly in the vinyl sleeves. Runner up is the X Movie box (JP) — I don't much like the front-&-back style boxes to begin with, & half the depth of this one is a foam pad. If you took it out, you could easily fit in your X2 & Tokyo Babylon discs.

15) Best Audio Commentary
16) Worst Audio Commentary
17) Best Supplements Third place has a lot of competition, but I'd say maybe the Roan "Fantastic Planet" (US) — an entirely separate film! Second place belongs to the Nadia box (JP), because of the omake films. But the combination of soundtrack album & production art stills on the Honneamise Memorial Box is still king.

18) Worst Supplements the El-Hazard AC-3 box (JP), just a collection of TV spots

19) Best Remaster
20) Worst Remaster
21) Best-Chosen Side Breaks
22) Worst Side Breaks Tenchi Muyo Episode 13 (US) — actually a disc break, which should only be a side break, & very awkwardly timed

23) Favorite Movie On Disc, Regardless of Presentation Quality
24) Favorite Guilty-Pleasure Movie on Disc (Perhaps not a good movie in itself, but one that makes a great laserdisc) perhaps The Ultimate Teacher (US)

25) Movie You Are Most Embarrassed To Admit That You Own possibly (since I got rid of MD Geist and The Humanoid) Battle Royal High School (US) — hard to believe AnimEigo released this stinker

26) Worst Case of Laser-Rot You Have Experienced
27) Approximate Number of Laserdisc Titles In Your Collection 500
28) First Laserdisc Ever Purchased Tenchi Muyo Episode 1 (US)

29) Movie You Own The Most Editions Of (Any video format) Gunbuster (3 LD editions) or Wings of Honneamise (US LD, JP LD, US BD/DVD combo)

30) Rarest/Most Valuable Disc Owned Possibly Imperial Wedding Ceremony (MUSE)
31) Favorite Laserdisc In Your Collection
32) Most Wanted Laserdisc Title (Actual releases only)

Well, I've updated my response. Not too many new answers, but one of these days to be sure.

Truly unwatchable blown up.

You can't count that, I can make anything look horrible blown out and up zoomed in etc.

Also amazing I've never seen this thread before, no time to do any filling out.
